Waiver and Release Form for Program （12285266 Canada Association）

In consideration of the acceptance of my application for 17 Basketball Club Program, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages or which may hereafter occur to me as a result of participation in said event. This release is intended to discharge in advance 17 Basketball Club, its employees, volunteers and agents from liability.

It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on my heirs and assignees.

In case of emergency or illness, every effort will be made to contact the parents or guardians. In the event that contact cannot be made, I agree that in case of an emergency or illness, a qualified medical physician may attend to my child.

The undersigned hereby grants 17 Basketball Club (12285266 Canada Association) permission to take and use still photographs and moving videos in the website and for display in the facility.

I confirm that I have read and fully understand this waiver and release of liability. I sign this waiver and release of liability voluntarily without any inducement, assurance, or warranty being made to me.
